Form and Map Demo

   This section demonstrates a form that is checked for completeness when you click on the "Map Yourself!" button (try it). If it is complete (enough), then a map of the address entered is retrieved from Google and the name and phone number from the form is placed above the map. Note: This is just a demo, none of the information entered on the form is kept or used in any other way.
